Q:Can I call Dr. Murakami on the phone like I used to?
A: If you have had prior communications with Dr. Murakami, he likely has a file of notes on your inquiry already established. You may contact him through email or through the number he may have left with your for further correspondence. Any new inquiries are being asked to fill out a health questionnaire.
Q: Why do I have to fill out a health questionnaire now? A: We have implemented the new Health Questionnaire to alleviate the numberÂ of hours Dr. Murakami spends on the phone with health inquiries. By having you pre-fill out this form, he saves approximately 1 hour of his time. Multiply that by an average of 5 such inquiries in a day, and he's saving 5 hours on the phone! The questionnaire is the same series of questions he would ask you on the phone.
Q:What if I don't have access to
a fax machine?
A: We are currently working on getting a fill-out-online-and-submit version of our questionnaire up for your convenience. Please watch for this new addition shortly!
Q: I can't seem to download the Health Questionnaire.
The Health Questionnaire is an Adobe Acrobat (pdf) document. This is a very common, standard file format for secured documents (meaning no one can alter them). You will need Adobe Acrobat to view and print any files ending in .pdf.
This free software is available online from the official Adobe site. It commonly comes installed for free on your computer when you purchase it. If you don't have it, you can access the free Adobe Reader download here
Q: I'm having a hard time filling in all the information on the Health Questionnaire.
A: We realize the length of the questionnaire can be somewhat intimidating, however, the more information Dr. Murakami has about your health history, the better
he can fully understand your current health inquiries. Fill in the information as best you can. If you need assistance due to memory problems or motor skills associated with Lyme symptoms, have a friend or family member help you.
Q: How long before I hear back from Dr. Murakami?
A: Once your faxed questionnaire is received, it is reviewed and highlighted for areas of concern. Dr. Murakami will then call you to further discuss the highlighted areas and to give you his opinion on your inquiry. All inquiries are handled by priority sequence.
